Transitioning from Pediatric Care to Adult Primary Care

Prepare your child for the transition from a pediatrician to an adult provider. As your child is growing up and moving towards adulthood, prepare them for the transition to an adult provider.

Here are some tips to prepare: 

  • Begin the conversation with your child and their pediatrician when your child is between the ages of 12-14.
  • Let your teenager guide the conversation and include them in all discussions with their providers to prepare for their independence.
  • Schedule your transition appointment with your pediatrician.
  • Come prepared to the transition appointment with questions on your child's medical needs, appointment records, medical records, medications and insurance.
