Accountable Health Communities (AHC) CHRISTUS IS HERE FOR YOU

Background

Have you ever been worried about not having enough food to feed your family? Concerned about not having enough money to pay this month's rent? Are you and your family experiencing difficult times and could use a helping hand? Are you needing assistance with your medical condition beyond the scope of a doctor's office? Do you receive Medicare or Medicaid?

If you answered yes to any of these questions, The Accountable Health Communities Program of CHRISTUS Santa Rosa is here to help you. 

CHRISTUS Santa Rosa Health System is one of 32 organizations in the nation to receive a prestigious grant from the Centers for Medicare and Medicaid Services (CMS) to implement the AHC Initiative. Over a five-year period, CHRISTUS will link Medicare and Medicaid beneficiaries with community services that may address their health-related social needs (i.e., housing instability, food insecurity, utility needs, interpersonal violence, and transportation needs).

By connecting our patients to community resources they need, the AHC aims to improve health outcomes, reduce the risk of developing chronic diseases, avoid unnecessary trips to the emergency room, and provide enhanced care to our patients.
