Online Allied Health Orientation

Print, sign and date this form and return via fax to the Medical Staff Services Department at 409-923-1888.

This form must be completed by indicating the dates of completion for each element.

Failure to return the signed Orientation Certification of Completion to Medical Staff Services will result in a delay in the appointment process, which may include being unable to complete your application.
