Meet Candace Montoya, CNP

Candace Montoya, CNP, is a board-certified nurse practitioner dedicated to delivering experience-driven, patient-centered care to adults 18 and older in the Portland community. With years of experience in family medicine, Candace brings both clinical expertise and genuine compassion to every patient she cares for.

Driven by her goal to empower her patients, she ensures that each patient receives education, prevention, and personalized care during every visit. From managing chronic conditions, seeking preventive care, or simply working toward a healthier lifestyle, Candace takes pride in listening, supporting, and guiding patients throughout their health journey.

She is honored to care for the people she grew up with, having been both born and raised on the Texas coast. After graduating from Incarnate Word Academy, she earned an associate's degree in nursing. Channeling her passion for helping others, she pursued a master’s degree in nursing from Texas A&M University - Corpus Christi. She remains committed to the highest standards of care, maintaining board certifications and is an active member of the Coastal Bend Advanced Nurse Practitioners and the American Association of Nurse Practitioners.

Outside of the clinic, Candace continues to support her community and loves to learn about the rich history of Corpus Christi and retelling tales to those who are interested. She loves to support her teams, most notably the Astros and games for her alma mater. She lives by her Catholic faith, which reinforces her calling to serve others with integrity, empathy, and humility.
