FAQs
How do I get copies of my medical records?
In most cases, you have the right to look at or obtain a copy of medical and billing information. The actual patient must present at the Release of Information office and complete a medical record request form and provide photo identification. A parent or legal guardian may request the records of their minor child.
Can someone pick up my medical records for me?
Medical records are release to the patient of record and/or their legal guardian (s). If you wish to have someone pick up your records, their name and address must be listed on the medical record request form. They will have to provide photo identification in order to receive the records.
Will I have to pay for copies of my medical record?
We may charge a fee for the cost of copying, related supplies or postage at the rate of $1.39 per page. The fees are established by the state of Texas and updated regularly.
How long will it take to copy my medical records?
On average, it should take approximately 30 minutes depending on the number of customers waiting.
How are the copies provided?
Medical Records are provided on paper and radiology images are provided on a CD.

COVID-19 Response
At this time, the HIM Department is not open to walk-ins. Please call the HIM Dept/Release of Information team before you come to the facility in an attempt to get copies of your records. During these times of social distancing, we are almost always able to provide what you need without you actually coming to the hospital. We can mail you the authorization to release protected health information form, you can mail it back to us, etc. If you need records sent to another provider, we can send them directly to that provider without you coming the hospital in person. If, after speaking with us, you must come to the hospital to complete your request, we can work with you to provide contactless delivery of copies, etc. Please call us first.
How do i get copies of my medical records?

A parent or legal guardian may request the records of their minor child. HIM only releases medical record copies. You should contact the Billing department directly for copies of statements or bills. You should contact the Radiology department directly for copies of images.

How long will it take to copy my medical records?
On average, it should take approximately 30 minutes to walk in and receive copies of your record. This depends entirely on the number of customers who arrived before you and the size of the record you are requesting. For large records or records that are mailed to requestors, processing time is 5-7 days.
How are the copies provided?
Medical record copies are provided in paper form, electronic form, or via secure portal, at the discretion of the requestor.
Where do I go to request and obtain my medical records?
Records for St. Frances Cabrini Hospital, CLIC, Pineville and Alexandria Community Clinics, Incarnate Word Specialty Clinic and the Cabrini Cancer Center are released through the HIM Dept of St. Frances Cabrini Hospital.
