Our Approach

The CHRISTUS Outpatient Diabetes Education Center is an Accredited Recognition Program with the American Diabetes Association.  Our outpatient diabetes self-management education/training (DSME/T) program is performed one on one with patients through a series of four classes with a clinical dietitian and nurse educator. The program provides a series of classes to educate patients about the performance of frequent self-monitoring of blood glucose, nutrition and exercise, medication and insulin treatment plans developed specifically for them and motivates patients to use the skills learned to enable self-management. Nutritional counseling focus can also be provided under the guidance of a registered dietitian for the purpose of managing diabetes.

Patients can enroll in the class through any physician referral.
