Online Medical Staff Orientation was designed with convenience in mind, and is your opportunity to complete requirements linked to the initial application process.

Once you have reviewed all this material, please fill out the Certification of Completion. This form must be completed by indicating the dates of completion for each element.
Sign and present the form to Medical Staff Services prior to your appointment date.
Certification of Completion
Print, sign and date this form and return via fax to the Medical Staff Services Department at 409-923-1888.

HPF and Dictation System - Call the Health Information Management Department at 409-892-7171, extension 4319, to schedule training.
Failure to return the signed Orientation Certification of Completion to Medical Staff Services will result in a delay in the appointment process, which may include being unable to complete your application.
In addition, your CHRISTUS Southeast Texas Health System identification and parking badges will only be issued following receipt of the signed orientation certification form.
