The Joint Commission, AAAHC, and CMS standards require us to verify that all staff members, including contract workers, are qualified and competent for the scope of care, treatment services they are permitted to provide. - You must submit all completed forms and required documents ( Job Role Packet and PSHP General Orientation Checklist) in one packet to CHRISTUS Spohn Human Resources at the contact information on the cover page of your application. Incomplete packets will be rejected and re-submission will be required.
- Failure to submit a complete application and fee will result in delays in processing.
- There is a non-refundable processing fee of $175 for Initial applications and $50 for Annual re-certification.
