The questions I asked weren’t documented but irrelevant things that weren’t given any medical advise on shouldn’t be listed. He copied the notes from my last visit and edited portions. Templates are fine to use but if they aren’t what I’m there for then they shouldn’t be copied.

Dr.Barker is the best heart Dr.
Don't let the outside of a person define how you address them. I was not feeling well and so I haven't been able to be my best. You might allow a person to get a word in edgewise on occasion. I know your time is valuable but so is mine. And I'm paying for the privilege to talk to you.
