Medication Management Program

CHRISTUS Health is excited to present to associates and eligible dependents the Diabetic and Rheumatology/Inflammatory Disease Care Management Program, designed to reduce your medication cost while also providing you with additional care management resources.

Associates and their dependents over the age of 18 who are enrolled in the CHRISTUS Associate health plan can receive their qualified medications at no cost through the mail. To receive eligible medication at no cost you must first participate in a free care management telehealth visit with a CHRISTUS Health clinician.

This visit is to explore your degree of diabetes and/or inflammatory condition management and assist you in getting these high-cost medications free of charge. If you are on any of the qualified medications listed below a representative will contact you to enroll, schedule a telehealth visit and provide detailed information about the program.

Enroll in the Program

To initiate the program and start receiving your eligible medication at no or lowered cost depending on your plan, you must participate in a free telehealth visit with a CHRISTUS Health clinician. This interaction is not meant in any way to affect the care you receive from your regular physician. This visit is to assist you in getting these high-cost medications free of charge.

To enroll, please call 1-800-214-7351 to speak to a representative who will give you more information about the program and explain how to schedule a telehealth visit.

Any of your other medications will still be covered under your current pharmacy benefit coverage. However, to receive the medication listed above at no cost, you must have a telehealth visit with a program clinician and have the medication dispensed by a participating CHRISTUS pharmacy.

CHRISTUS is excited about this opportunity, which will help our Associates and their families save money and increase their access to medical support. We anticipate that this program will save Associates approximately $870,000 a year and provide cost savings to our health plan as well. We are proud of this first-of-its-kind program at CHRISTUS Health and look forward to seeing all the benefits it brings to our Associates and their loved ones on their health care journeys.

Requirements to Participate

The Medication Management Program is mandatory and will save Associates money, but please be advised, if you do not participate in this program you will have to pay full price for the medications listed above after the deadline.

Records Request: If your care provider is a member of a CHRISTUS physician group, this step isn’t required.

In order to request your medical records from your Primary Care Physician, you will need to download this pre-filled records request form, sign it, and send it to your PCP. This step is very important and will greatly help in streamlining your appointment process. Click here for a records request.

Schedule Appointment: In order to schedule an appointment, you need an Epic MyChart account. For activation of this account, you can call the program office at 1-800-214-7351.
