Schedule Your Mammogram

1 out of 8 women will receive a breast cancer diagnosis in her lifetime. At CHRISTUS Health, we believe even one is too many. Scheduling your annual mammogram and consulting with your primary care physician about preventive care measures establishes a foundation for optimal breast health and wellness.

Fill out the form to request a mammogram at a CHRISTUS Health location near you, or call the location's number below.

Request a Mammogram

Mammograms can detect breast cancer early, when it’s most treatable. Fill out this form or call 888-802-0410 and a CHRISTUS representative will reach out via phone or email within 1-2 business days.
